Overview

You can download or print your member ID card even if you're not registered in My Plan or don't know your member ID number.

Download Instructions

Step 1: Visit the Website

• Go to hpiTPA.com

Step 2: Navigate to Members Section

• Select "Members" from the Your Resources menu

Step 3: Access ID Card Page

• Click "Member ID Card"

Step 4: Enter Your Information

• Enter your information under "Download and Print Your Card"
• For Dependent Cards: To access a dependent's card, enter:
  • The dependent's name and date of birth (DOB)
  • The plan subscriber's social security number

Step 5: Submit and Download

• Click Submit and a PDF will open in a new window
• Your actual card image will vary (the example shown is for illustration only)

Understanding Your Member ID Card

Your member ID card contains important information organized into several sections:

Member Information

• ID Number: Your unique member identification number
• Group Number: Your employer or group plan identifier
• Member Names: Primary subscriber and all dependents listed

Medical Cost Share Information

• In-Network / Out-of-Network: Different cost structures based on provider network status
• Preventive: Preventive care cost sharing amounts
• Deductible: Annual deductible amounts (In-Network / Out-of-Network)
• OOPM: Out-of-Pocket Maximum amounts (In-Network / Out-of-Network)

Provider Network Information

• Your Provider Network Logo: Visual identifier for your specific network
• Network Access Description: Information about how you may access your network

Contact Information Sections

For Members:

• Website: hpiTPA.com
• Phone: Customer Service number (listed on card)

For Providers:

• Precertification: Providers must FAX all Precerts to the number listed on the card
• In-Area Network Claims: Information about where providers can submit claims within the area network
• Out-of-Area Network Claims: Information about where providers can submit claims outside the area network

Prescription Plan Information

• Pharmacy Benefit Questions: Dedicated Pharmacy Help Desk phone number
• Network Access Pharmacy: Details about accessing pharmacy network

Sample Information (Example Only):

• ID#: HHZ010000
• Group: Z01
• Members: SMITH: THOMAS, NICOLE, WILLIAM, SAMANTHA, TYLER
• Employer: ABC COMPANY

Important Notes

• Your actual card image will vary from the example shown
• Keep your member ID card accessible for healthcare appointments
• Both digital and physical cards are valid for use
